This is an umbrella concept spanning multiple independent communication models. It names the phenomenon cause-agnostically; individual frameworks (Shannon-Weaver, Schulz von Thun, Hall, Clark) each diagnose specific causes. Encoding and decoding are separate processes

The sender encodes meaning into words, signs, or structure. The receiver decodes them through their own context, knowledge, and assumptions. Any mismatch between the sender’s encoding frame and the receiver’s decoding frame produces a discrepancy.

Insufficient common ground (Clark & Brennan, 1991)

Communication succeeds only when sender and receiver have accumulated enough shared knowledge, beliefs, and assumptions — their common ground. Without adequate grounding, the receiver fills gaps with their own model rather than the sender’s intent.

Meaning is reconstructed, not transmitted

No meaning travels intact from sender to receiver. The receiver always re-creates meaning by combining what the message provides with what they already know. The illusion of transparent transmission is itself a source of discrepancy.
